Vision vs. Medical Coverage
Your exam may be covered by a vision plan or by your regular medical plan. If it is a routine annual exam your vision plan will apply. If it is a visit for any non-routine concern your medical plan will apply.
Privacy Notice
As a patient, you have the right to adequate notice of the uses and disclosures of your protected health information. Under the Health Insurance Portability and Accessibility Act (HIPAA), RVA Eye Care Optometrists can use your protected health information for treatment, payment and health care operations. Intake Form
Use the button below to view our intake form. Please fill it out and print it to bring to our office on the day of your exam. It is needed for all visits, and we are required to have a new one each year.
