Information for patients

Patient Resources

Prepare for your visit, understand your coverage, and find answers about appointments, prescriptions, and eye care.

Prepare for Your Visit

Having the following information available helps your doctor and office team prepare for your care.

  • Photo identification and current contact information
  • Medical and vision insurance cards
  • Your current glasses, including reading or computer glasses
  • Contact lens brand and prescription information, if available
  • A list of medications, eye drops, and allergies
  • Relevant eye history, previous procedures, and family eye conditions
  • Questions or changes in vision you want to discuss

For a first visit, ask your office whether any records or paperwork are needed in advance. If you need language or accessibility assistance, contact the office before your appointment.

Your Eye Examination

What can I expect during an eye examination?

Your doctor will review your concerns and health history, assess your vision, and examine your eye health. The tests recommended will depend on your symptoms, history, and individual needs.

Will my eyes be dilated?

Your doctor will discuss whether dilation is recommended to examine the inside of your eyes. Dilation can temporarily blur vision and increase sensitivity to light. Bring sunglasses and arrange for someone to drive you home if your eyes will be dilated. Read the National Eye Institute’s guide to dilation.

How often should I schedule an eye examination?

The recommended interval depends on your eye health and risk factors. Ask your doctor when to return and whether any follow-up visits are needed.

What should I mention to my doctor?

Discuss changes in vision, discomfort, current glasses or contact lens concerns, medications, and any changes in your general health. Include the visual demands of your work, reading, or other daily activities.

Insurance & Billing

We do not participate in Medicaid. Please contact your office with questions about accepted insurance plans.

Bring both medical and vision insurance information. The coverage used for a visit depends on the reason for the examination, services provided, and your plan.

What is the difference between vision benefits and medical insurance?

Vision benefits generally apply to routine vision care. Medical insurance may apply when a visit addresses an eye health concern. Your plan determines coverage, eligibility, and out-of-pocket costs.

How do I confirm whether my plan is accepted?

Contact the office with your insurer and exact plan name before your visit. You can also ask your insurer about network status and eligibility; plans from the same insurer may have different networks.

What costs should I ask about before my visit?

Ask about copays, deductibles, examination benefits, and any separate contact lens evaluation or eyewear costs. Your office can explain the proposed services, and your insurer can clarify plan benefits.

Who can help with a bill or insurance question?

Contact the office where you received care. Have the date of service and billing reference available when you call. Please do not send insurance identification numbers through the general contact form.

Contact Lens Care

Follow your doctor’s instructions for your specific lenses and care products.

  • Wash and thoroughly dry your hands before handling lenses.
  • Remove lenses before showering or swimming, and keep them away from water.
  • Use the recommended lens care solution. Do not add fresh solution to used solution.
  • Follow the prescribed replacement schedule and avoid sleeping in lenses unless your doctor directs you to do so.
  • Keep backup glasses available. If lenses cause discomfort, remove them and contact your eye care provider.

What should I bring to a contact lens appointment?

Bring your current lens brand and prescription details, glasses, and any relevant care products. Ask your office whether you should wear your lenses to the appointment.

How do I order or check on contact lenses?

Call the office that manages your contact lens prescription. The team can help with prescription status, ordering, and updates on an existing order.

Eyewear & Prescriptions

How do I check the status of my glasses?

Contact the office where you placed the order. Your optical team can provide an update and explain collection arrangements.

Can I get help with glasses that need adjusting or repair?

Call your optical team to discuss the issue and arrange a visit. Bring your glasses so the team can assess the fit or damage and explain available options.

How do I request a copy of my prescription or records?

Contact the office where you were examined. The team can explain how to request your prescription or records and any identity verification or authorization needed.

What if my new glasses or contacts do not feel right?

Contact the office that provided them and describe what you are experiencing. The team can advise you on the appropriate next step or arrange follow-up.

Appointments & Forms

How do I request an appointment?

Use the Schedule an Appointment page and select your office. Its Weave booking page opens in a new tab. The office will confirm your appointment details.

How do I change or cancel an appointment?

Call the office where your appointment is scheduled. Please provide as much notice as possible and ask about the office’s current cancellation policy.

Are new-patient forms available?

Contact your office for the current forms and instructions for completing them. If you receive a request for paperwork before your visit, follow the instructions provided by the office.

Can I request a particular doctor or book a visit for my child?

Contact your preferred office to confirm doctor availability and the appropriate appointment type. For a child’s visit, provide their age and any specific concerns when scheduling.

For urgent eye concerns, call your office directly rather than using an online appointment request or general message form.
