How this website handles information.
General office messages
The contact form collects your selected office, topic, name, email address and message so that our office can respond. Messages are sent through Microsoft 365 to the selected office and a sent copy is retained in the website mailbox. The website does not store submissions in a separate application database.
Please do not submit symptoms, medical concerns, emergencies, insurance identification numbers or other sensitive health information. Call your office for clinical questions.
Technical information
The hosting service processes technical information needed to deliver and protect the website, such as request details and IP addresses. The form uses temporary request counts and a hidden field to help limit spam.
Analytics and external links
This website does not currently include Google Analytics or advertising tracking scripts. Scheduling opens Weave; directions and reviews open Google. Those services have their own privacy practices.
